    Patient Guides

    The following patient guide is available to help you and your family understand what to expect before, during and after spine surgery at AAMC. Please remember that this guide is not meant to replace discussions with your surgeon or other health care providers.

    Your Visit

    For us to best serve you, it’s important that you bring these items to your visit:

    • Photo ID
    • Physician referral (otherwise, you will have to reschedule your appointment)
    • Your health insurance card and copay*
    • List of current medications
    • All previous X-ray, MRI, CT or bone scans
    • Your referring physician and primary care physician’s name, address and phone number
    • List of questions and concerns
    • Parent or legal guardian, if the patient is under the age of 18

    About MyChart

    My Chart offers you secure, easy-to-use and free online access to portions of your medical records. Your patient login allows you to view test results and a health summary, request prescription renewals, access trusted health information resources, and communicate electronically and securely with your medical care team. Sign up for or log on to MyChart.

    We also offer video visits through our MyAAMC app, available in the Apple store and Google Play. This service is a convenient way to check-in with your doctor after surgery, or get help fast from our partners at AAMG Physical Therapy.
